Child Health and Disability Prevention (CHDP)
The County of Santa Clara Public Health Department operated the county's Child Health and Disability Prevention (CHDP) program. Funded by the State of California, CHDP provided for the early detection and prevention of health problems among children and youth from low-to-moderate-income families. As of July 1, 2024, CHDP services will be provided by other Medi-Cal delivery systems or services and no longer through Public Health.
CHDP transition
Santa Clara County CHDP is no longer accepting new enrollments. Services that will continue are
- Presumptive eligibility enrollment activities currently offered through the CHDP Gateway
- Activities under the Childhood Lead Poisoning Prevention Program (CLPPP)
- The Health Care Program for Children in Foster Care (HCPCFC)
Children's Presumptive Eligibility (CPE)
- Children’s Presumptive Eligibility (CPE) gives temporary, free Medi-Cal coverage to children and youth ages 0 to 21 years from low-income families.
- Under CPE, a family can quickly and easily enroll their child in temporary, full-scope Medi-Cal based on a simple form. The family still needs to file a full Medi-Cal application to check that they qualify to keep the coverage. But, during that time, CPE allows children and youth to get the care they need right away.
Care coordination
As of July 1, 2024, patients need to contact their health provider or Medi-Cal Managed Care Plan (MCP) for care coordination.
