Ryan White Program
Effective July 1, 2024, the County of Santa Clara’s Public Health Department, Sexual Health and Harm Reduction Program will be providing robust case management services to Santa Clara County residents with low income who are living with HIV.
To be eligible for the Ryan White Program, you must have a diagnosis of HIV or AIDS, live in Santa Clara County, and meet all financial qualifications.
We are accepting appointments which are required for most services.
Some of the services we offer include:
- Screening, enrollment, and re-enrollment in the Ryan White Program and AIDS Drug Assistance Program (ADAP)
- Medical and non-medical case management services, including comprehensive assessments and service plans
- Medical transportation services and emergency financial assistance
- Ongoing support in maintaining medical care, care coordination, and treatment adherence counseling
- Linkages to primary health care, mental health, food and housing assistance, drug and alcohol treatment, dental care, legal services, and support groups
Current Ryan White clients
As a current client, you will not need to reenroll in the program. Your current enrollment will remain active until your next re-enrollment date.
Anyone with a re-enrollment date after July 1, 2024 will complete their re-enrollment with a case manager at Public Health. Your new case manager will contact you before your re-enrollment date using the contact information you provided to The Health Trust. Your information will automatically transfer over from The Health Trust once you agree to receive case management services from Public Health.
Housing
- Housing services funded by the Ryan White Program, also referred to as Housing Plus Project or HPP, will be provided by Bay Area Community Health. Appointments will be required. Learn more about Bay Area Community Health.
Food
- Starting July 1, 2024, food services previously received through The Jerry Larson Food Basket will be provided by Loaves and Fishes. Clients currently accessing food services will not need to re-establish eligibility to begin receiving the same services with the new provider, but you may need to consent to receive services from this new agency during your pickup.
- Beginning July 2024, Safeway food vouchers will be distributed to eligible clients by the Ryan White Program at the Public Health Department as the new case management organization.
- Housing services funded by the Ryan White Program, also referred to as Housing Plus Project or HPP, will be provided by Bay Area Community Health. Appointments will be required. Learn more about Bay Area Community Health.
Medical transportation
- Medical transportation services will continue to be provided through case management staff, now within the Ryan White Program at Public Health. If you were already receiving a VTA bus pass, they will be available for pickup after the 18th of each month.
