Transitioning out of California children's services
At 18 and 21 years of age, your child/youth will have special needs. At age 18, your child becomes an adult. At age 21, your child can no longer participate in CCS. We want to make sure we support you and your young adult during these transition times. We will provide you with a program that will best meet your child’s needs at this stage of their life. Below are some guidelines and resources to help young adults stay in good health.
Important questions:
- When will my pediatric doctor stop seeing me? Will my doctor continue to see me as a young adult?
- Who will be my adult doctor(s)?
- What medication, equipment, or supplies do I need? And where do I get them?
- What health insurance and/or income programs will I be able to get?
- What skills and information do I need to keep myself in good overall health?
- How will my health needs fit with my plans for the future: education, work and living on my own?
- Are there activities or groups in my community, especially for people with my health condition?
These are just a few questions. It is important to include your family and other caregivers in your transition planning. It is very important that you and your caregivers be prepared when you turn 18 or 21 years old.
Our staff will contact clients 6 months before their 18th and 21st birthdays to schedule a transition meeting. During this meeting, we will go over the transition process and provide resources to help with the transition. You can call us at any time to learn more about the transition process.
