Lost your vaccine records?
- Digital copies are available from the Digital Vaccine Record Portal for free.
- Call your doctor’s office or clinic where you got your most recent shots.
- Hard copies of the California immunization record (yellow card) can be provided at our clinic for $5.
Lost your Yellow Fever Vaccination Certificate?
- If you received your vaccination at our clinic, replacements are available for $10.
- If you received your vaccination at another doctor’s office or clinic, you will need to reach out to them directly for a replacement.
