What is the CHA?

​The Community Health Assessment (CHA), a process facilitated by the County of Santa Clara Public Health Department, Community Health Partnership, and other community partners. The CHA is a compilation of new and existing data sources that collectively capture our community health profile, and were used to inform the prioritization and identification of five health issues.

What is the CHIP?

​The CHIP is an acronym for the Community Health Improvement Plan. The CHIP provides a framework for addressing issues identified by the CHA to improve the health of the County of Santa Clara residents by using the data to develop and implement strategies for action and establish accountability to ensure health improvements.

What is the MAPP Framework?

​Mobilizing for Action through Planning and Partnerships (MAPP) is a community-driven strategic planning process for improving community health. MAPP provides a structure for convening a variety of organizations, groups, and individuals to create and implement a community health improvement plan. The framework is rooted in the principles of health equity. The MAPP process has 6 phases, each building on one another, and the interconnectedness of the planning processes resulted in the development of a combined CHA and CHIP report.

How will this effort build on existing efforts in the County of Santa Clara?

This effort will incorporate existing data collected by community partners and engage in joint data collection efforts. The steering committee will set health priorities and develop strategies​.

Why should community members get involved?

Public health is a multi-faceted system consisting of many diverse agencies, stakeholders, and community residents; working together we each have a role in improving the health of the County of Santa Clara.​
