Partners for Health Santa Clara County is a cross-sector partnership formed to synchronize efforts to improve community health and achieve better health for all. It is guided by the strategic priorities identified through a Community Health Assessment (CHA) and community health improvement planning (CHIP) process, which took place from 2012-2014. Each organization represented in the partnership works collectively to develop, prioritize, and implement strategies to ensure measureable health improvements.
The vision of Partners for Health is that Santa Clara County is a community united to eliminate health disparities and dedicated to creating healthy environments, positive health outcomes, and optimal quality of life for all. The goal of Partners for Health Collaborative is to provide a platform to create linkages, identify synergistic efforts, discover opportunities for collaboration, celebrate successes in five prioritized areas selected by community stakeholders:
